What is Shockwave Therapy ESWT?

Shockwave therapy machines pass short, intense energy sound waves, travelling faster than the speed of sound, into affected tissues. This causes microtrauma, increases blood flow and stimulates an acute inflammatory response which aids the healing process.

Shockwave therapy is widely used in physiotherapy, podiatry, sports medicine, urology and orthopaedics to provide fast pain relief and increased mobility. ESWT (Extracorporeal Shockwave Therapy) is increasingly used as a treatment for health problems such as erectile dysfunction, slow-healing wounds, cellulite and neurological conditions.

Shockwave therapy has both NICE guidance and FDA approval for a large number of indications. If delivered by qualified therapists, the treatment is largely risk and side-effect-free, making it a safe treatment choice for musculoskeletal conditions and chronic pain. Shockwave therapy is a highly trusted treatment modality, and is used by hundreds of private clinics and NHS hospitals across the UK.

The Many Benefits Of
Shockwave Therapy:

  • Fast recovery
  • Fewer potential complications
  • No anesthesia
  • Non invasive
  • No medication
  • Fast treatment: 20 minutes per session
  • Significant clinical benefit: often seen 6 to 8 weeks after treatment

Two Types of Shockwave Therapy

Shockwaves can be either focused or radial.

Radial shockwave therapy machines (also called radial pressure wave) produce pressure waves that are strongest at their source (the handpiece) and spread out into the tissue. The further they travel, the weaker they become and they can reach treatment depths of up to 5cm. Radial shockwaves perform on greater surface areas of application or larger regions of the body.

Focused shockwave therapy machines produce sound waves that are gathered (or focused) into deeper regions of the body, approximately 12.5cm. It is at this point that they reach their maximum energy. Focused shockwaves are used in the treatment of deep trigger points or areas, whereas radial shockwaves transmit outwards and do not reach as deep into the target area.

The type of shockwave therapy machine best for your clinic depends on the patients you see and the conditions you are treating. Some therapists will use a combination of both for maximum effect.

Two Technologies – Two Ways of Generation

Choose Your Technology

Focused

Higher energy density | Regenerative

focused shockwave therapy machine handpiece

Focused shockwave therapy is ideal for:
Diffuse Pain Presentation / Acute pain & Flare Ups
Insertional Tendinopathies / Deep-Seated Tendons
Degenerative Pathologies / Bone stress Injuries / Stress Fractures / Non-unions / Osteoarthritis / Spinal Facet Joint Pain / Coccydynia / Carpal Tunnel / Men’s Health – ED / CPPS / Wound Healing

Radial

Lower energy density | Pro-Inflammatory

Radial shockwave therapy is ideal for:
Localised Pain Symptoms / Chronic and Stable Pain
Large muscle groups and muscle disorders / Plantar Fasciitis
Myofascial Pain Syndrome / Chronic Tendinopathies
Superficial treatment depths / Larger treatment areas

What can be treated with a shockwave therapy machine?

Shockwave therapy has become widely popular and accessible as it is clinically proven to provide fast and .effective results, along with the added bonus of no medication and it’s non-invasive nature.

Many injuries and disorders have been effectively treated with the use of shockwave therapy, including, but not limited to:

Common indications

  • Jumper’s knee
  • Painful shoulder
  • Tennis elbow
  • Golfers elbow
  • Heel spur
  • Plantar fasciitis
  • Insertional pain
  • Chronic tendinopathy/chronic pain
  • Medial tibial stress syndrome
  • Calcifications
  • Hip pain
  • Achilles tendinitis
  • Myofascial Pain Syndromes
  • Musculoskeletal conditions (you can find specific facts and stats here.)
  • Calcific tendonitis
  • Hamstring tendinopathy
  • Non healing wounds
  • Non-unions
  • Coccydynia
  • Erectile Dysfunction
  • Peyronie’s disease
  • Chronic Pelvic Pain Syndrome

How much do shockwave therapy treatments cost?

The cost of shockwave therapy varies from clinic to clinic and depending on the exact type of shockwave treatment required and if the therapy is combined with other treatments. As a general guide shockwave therapy ESWT sessions start anywhere from about £60-£100 per session, with a lot offering discounts the more sessions you book.

Some types of extracorporeal shockwave therapy are also available on the NHS, although the waiting list for treatment can be long.

What are the Therapeutic Effects of Shockwave Therapy?

The therapeutic effects of shockwaves on the musculoskeletal system for pain relief and healing are based on host cellular and molecular mechanisms. Scientists fully recognise the valuable effects of shockwave treatments and how they trigger direct action adequately.

One of the central messenger substances that is released is known as Substance P, which is a growth factor and pain moderator. The discharge of such a substance produces an analgesic effect whilst amplifying the blood vessels, boosting blood circulation and contributing to the design of new bone tissue. In essence, we know that when treating an area of pain, shockwaves directly speed up the repair process.

By committing to the release of free radicals, shockwave treatments help to enhance our endogenous cellular defense mechanisms to protect us from disease. The body’s signal and modulator modules play a crucial role in the activation of these cells.

By suppressing inflammatory mediators such as Cyclooxygenase-2 (COX II), shockwaves create an anti-inflammatory reaction, which decreases any inflammatory process. In short, it is an important stimulant during the healing process.

Studies carried out by scientists have also shown that shockwaves act in another unique way – hyper stimulating the nerve fibres, this blocks any escalation in pain stimulation and therefore strengthens the analgesic effect (also known as Gate Control Theory).
